Pulled tight in the back, the waist measures 38 inches. The ties are very long, so this can be expanded well beyond that. However, the flap that covers your back behind the straps is about 5.5" long so if you want to be covered, your waist should cinch to about 43". The pair of underwear that goes with this is factory sealed in plastic. It looks like their bikini underwear but I do not want to take them out of the bag to photograph them. As far as I can tell, they are all lace and stretchy. I have no idea why they sized this the way they did so please go by the measurements and the photographs.
